We have centralized the essential forms you need to manage your employment and benefits.
Please find the forms below. Most forms are available as downloadable attachments; please ensure they are completed clearly and submitted to the Board Office according to the instructions provided on each document.
Submission Note: Please ensure all forms are signed where required. You may submit completed documents via email to payroll@tecumsehlcoal.org or drop them off in person at the Board Office.
Legal Name Change?
If you have recently had a legal name change, it is important to update your information with the District Office to ensure your payroll, benefits, and tax documents remain accurate.
To initiate this process, please follow these steps:

Obtain Legal Documentation: We require a copy of an official document verifying your name change, such as a Updated Social Security Card, Updated Driver’s License, or Court Issued Documentation.

Submit a Name Change Request: Please complete the official Name Change Form (click the link below). You can upload all documentation via the form.
Important Considerations:

Payroll & Taxes: Your name on file must match the name registered with the Social Security Administration (SSA) to avoid issues with your W-2 and tax filings. We recommend updating your information with the SSA first.

District Accounts: Pleae note that your district email AND ProgressBook account will NOT be updated until the form is completed and verifying documents received.

Benefits: Once your name is updated in our system, we will notify your insurance providers to update your coverage records.
