Vaccination Clinic & Sports/Health Physicals

The Vaccination Clinic is set up for the 2025-2026 school year. We are partnering with New Horizon Health Center in New Carlisle. As Kindergarten screening is approaching and we are preparing records for next year, please advertise this to your students and their parents.

New this year they will be offering physicals where they can receive them for sports and for their general yearly checkups. This gives us an opportunity to have the children screened earlier for ears and eyes and be a patient at this doctor's office, which then enables them to be seen by the vision and dental teams throughout the school year.

When: August 18th, 2025

Where: Park Layne Elementary Gym – 12355 Dille Rd. New Carlisle, Ohio 45344

Time: 9:00AM – 2:30PM
